One of the more annoying aspects of aging, particularly on the face, is volume loss. Volume loss occurs primarily due to a decline in the body’s production of collagen that is a part of aging. Every year after we turn 20 our body produces one additional percentage point less collagen. That means by the age of 50 your body is producing 30 percent less collagen. And since collagen provides the skin’s underlying structural support, it’s no wonder our skin starts to sag and develop wrinkles and creases.

People confuse dermal fillers such as Restylane® with neuromodulators such as BOTOX. Dermal fillers only work on “static wrinkles.” Static wrinkles don’t have anything to do with muscle contractions; they are due to sun damage, declining collagen production, environmental factors, and personal habits such as smoking. Static wrinkles form on the mid to lower portions of the face.

At Laser & Mohs Dermatology of New York, we offer Juvederm Volbella® XC. This dermal filler is proven to increase lip fullness and soften the appearance of vertical lip lines for up to 1 year.
Restylane®
At our practice, we offer several Restylane products to improve the signs of wrinkles. These dermal fillers are a great option for those looking for skin rejuvenation. The Restylane products we offer include Restylane, Restylane Silk, Restylane Lyft, Restylane Refyne & Defyne. These Restylane products have different benefits including adding volume to cheeks, creating fuller lips, and filling in lines and wrinkles.
Sculptra®
Sculptra is PLLA, or poly-L-lactic acid. This is the same chemical that our muscles make after we exercise. This acid creates a temporary scaffolding to support superficial tissue and stimulates the production of new collagen. In contrast to standard dermal fillers that diminish after a few months, Sculptra continues to work beneath the surface, leading to younger-looking skin.
Radiesse®
Radiesse is a safe biocompatible substance that adds volume and shape to your face, defying wrinkles, and producing collagen at the injection site. What Are Neuromodulators?
How Do Neuromodulators Work?
Neuromodulators, work completely different from dermal fillers. Neuromodulators work on “dynamic” wrinkles. These are wrinkles that form due to muscle contractions under the skin. Dynamic wrinkles occur mainly on the expressive upper third of the face when you make certain expressions such as frowning and squinting.Neuromodulators work by blocking the nerve messages from the muscles to the brain. When injected into the muscle below a wrinkle, the neuromodulator blocks the nerve signal, so the brain never receives the command to contract the muscle. Without the contraction, the wrinkle on the surface doesn’t form. This paralysis lasts from four to six months before the body absorbs the now-inert botulinum toxin and the muscle will start contracting again and the wrinkle will return. At that point, another session receiving BOTOX®, Dysport®, or Xeomin® will be necessary to maintain your results.
Neuromodulators We Offer in NYC
We offer the three neuromodulators approved by the FDA. They are:

- Xeomin®
All three are very similar. Xeomin® is sometimes called the “naked” neuromodulator because it consists of only botulinum toxin, while BOTOX® and Dysport® also have some additional proteins in their formulations. If a patient has a subtle reaction to one neuromodulator, we can try one of the others.
Botox® in NYC
Dr. Quintana administers Botox Cosmetic to improve the looks of moderate to severe frown lines between the eyebrows. The purified protein in the injection works immediately on the facial muscles that cause wrinkles. You will be able to see the effects of Botox within a few days.

Dysport injections are a simple, effective, and non-surgical treatment to relax facial muscles on the forehead. With just one injection into each of the 5 points between and over the eyebrows, Dysport reduces muscle activity and temporarily prevents contraction of the muscles that cause frown lines. Dr. Quintana can even inject Dysport into the neck for wrinkles.
What Areas Of The Face Do Neuromodulators Treat?
Dermal Fillers vs. Neuromodulators
Sometimes our Laser & Mohs Dermatology patients clump all injectables together, but dermal fillers and neuromodulators are quite different.As described above, dermal fillers “fill” back in areas of volume loss. They are injected and add volume and push the wrinkle or depressed area back up. Dermal fillers are used on the mid to lower face and around the mouth. They also can return volume to the backs of the hands.
Neuromodulators work in a completely different fashion. Neuromodulators work by blocking the nerve messages that are sent from the muscles to the brain. After a muscle is injected with a neuromodulator, the brain doesn’t receive the message to initiate the contraction of the muscle, so the muscle stays relaxed. On the upper third of the face, muscle contractions form crow’s feet, the 11s between the eyebrows, and various forehead lines. BOTOX® and the other neuromodulators erase these wrinkles by stopping the muscle contractions under the skin.
What To Expect During Injectable Treatment
These sessions take just a few minutes, so they can be done over a lunch hour. BOTOX® and the other neuromodulators are injected with very tiny needles, so patients don’t usually request topical numbing prior to their injections. Plus, these products are not injected at great depth.Some dermal fillers may be injected more deeply, depending upon the area of volume loss. Patients sometimes request topical numbing cream prior to these sessions. The choice is completely up to you.
